Repeat Prescriptions

Patients on regular medication should, where ever possible, give two full working days' notice by either telephoning after 11:00 or calling into the surgery for a repeat prescription. A detachable counterfoil can be used when reordering your medication – simply tick the medication you require. If you want your prescription to be posted back to you, please enclose a stamped, addressed, envelope and allow one week for return.

Prescriptions can also be requested by fax on 0121 422 4808.

Prescriptions can also be requested online via the link at the top of this page. New users are required to complete a simple registration.

Prescriptions can also be requested by clicking here, downloading the form and filling it and bringing it into the surgery.

Medication Reviews

Patients on repeat medication will be asked to see a doctor, nurse practitioner or practice nurse at least once a year to review these regular medications and notification should appear on your repeat slip.

Please ensure that you book an appropriate appointment to avoid unnecessary delays to further prescriptions.
